Read this frame when the prescription is as-needed tadalafil. Open the daily glint when the bag is 2.5 mg or 5 mg every morning.
UK and US labels agree on the as-needed start: 10 mg taken prior to anticipated sexual activity. The UK SmPC says with or without food. The US Cialis label says the same in one short sentence. Maximum frequency is once per day.
If 10 mg is weak after a fair try, the labelled step is 20 mg. If 10 mg is rough, the labelled step is 5 mg. Those two moves are titration, not a hunt for a secret stronger brand.
Tadalafil 10 mg and 20 mg are intended for use before activity. They are not recommended for continuous daily swallowing. Daily erectile-dysfunction use lives on 2.5 mg or 5 mg at about the same clock each day. Mixing a daily strip with a leftover 10 mg film is how hypotension and Monday ache get worse.
Brand foil still says Cialis. Scottish bags more often say tadalafil. Same INN. Compare class clocks on the PDE5 duet. Zoom the two-evening question on the weekend glass glint.

Nitrates stay banned; CYP3A4 cuts the 10 mg start
Any organic nitrate or nitrite, regular or intermittent, is contraindicated. That includes GTN spray, isosorbide, and recreational amyl nitrite. Riociguat and other guanylate-cyclase stimulators sit in the same ban. This is not a 'usually avoid' hedge. The 17.5-hour half-life makes the nitrate conversation longer than a Viagra evening.
Strong CYP3A4 inhibitors - ketoconazole, itraconazole, ritonavir - raise tadalafil exposure. As-needed use then tops at 10 mg no more than once every 72 hours. Daily use tops at 2.5 mg. Those cuts are why 10 mg is a start, not a free ceiling, when the chart already holds a strong inhibitor.
Alpha-blockers are a caution, not a second contraindication. Stabilise the alpha-blocker first. Start tadalafil at the low end. Watch standing pressure. Solo 5 mg daily for BPH is allowed. Two vasodilators for the same lower-urinary complaint is the visit that goes wrong.

Creatinine and Child-Pugh trim the 10 mg start
Mild or moderate renal impairment roughly doubled tadalafil AUC after single 5-10 mg doses. End-stage kidney disease on haemodialysis raised Cmax about two-fold and AUC 2.7- to 4.1-fold after 10 or 20 mg. Dialysis between 24 and 30 hours after the dose barely cleared parent or metabolite.
As-needed use in CrCl 30-50 mL/min starts at 5 mg and tops at 10 mg no more than every 48 hours. CrCl below 30 or haemodialysis: 5 mg as needed no more than every 72 hours; daily use is not recommended. A headline that crowns 20 mg as the default weekend tablet does not survive that chart.
Mild-to-moderate hepatic impairment: 10 mg as needed is the usual ceiling of that conversation. Severe hepatic disease keeps daily use off the table and makes as-needed use a specialist call. There are no labelled data for doses above 10 mg in hepatic impairment.

Seventeen and a half hours, then the 36-hour claim
Mean terminal half-life is 17.5 hours in healthy adults. Mean oral clearance is 2.5 L/h. Apparent volume of distribution is about 63 L. At therapeutic concentrations, 94% of circulating tadalafil is protein-bound.
Those four numbers are why a Friday 10 mg tablet can still be in play on Saturday. They are also why a nitrate conversation lasts longer than one evening. The shorter oral PDE5 drugs clear on a four-hour clock. This one does not.
The 36-hour line is an efficacy window against placebo after as-needed dosing, not a promise of a day-and-a-half erection. Sexual stimulation is still required. Some men notice an effect near 30 minutes. Median Tmax sits near two hours, with a reported range from 0.5 to 6 hours.
Excretion is mostly as metabolites: about 61% in faeces and about 36% in urine. Morning versus evening swallow did not change the rate or extent of absorption in the labelled studies.

A 10 mg PRN film is not a daily 2.5 mg twin
Search pages keep treating every Cialis milligram as the same weekend story. The label does not. As-needed 10 mg is a single swallow before activity. Daily 2.5 mg is a floor taken at about the same time each day, with a step to 5 mg if needed.
The UK wording for frequent use - at least twice weekly - points toward a once-daily regimen at 5 mg, which may drop to 2.5 mg on tolerability. The US daily-ED start is 2.5 mg, which may rise to 5 mg. Either way, 10 mg and 20 mg are the as-needed pair, not a daily habit.
Do not add a holiday 20 mg to an ongoing daily 5 mg without a written plan. Once per day is the hard cap on either schedule. If the daily floor is flat, the next visit is a scheduled rise to 5 mg daily or a switch to as-needed, not a hidden second film.
Five milligrams daily is also the BPH dose and the combined ED-plus-BPH dose. That overlap is useful when both problems sit in one man. It is messy when a writer treats every 5 mg tablet as a Friday pill.

Five milligrams daily for BPH is a second claim
Once-daily 5 mg is the labelled dose for signs and symptoms of benign prostatic hyperplasia, and for men who have both ED and BPH. That is a separate efficacy claim with its own trials. It is not a free extra that appears when someone swallows a 10 mg Friday tablet.
A Dosage page that says Cialis is not for the prostate has misread the indication. A page that says add it on top of tamsulosin tonight has misread the caution. If LUTS improved on 5 mg daily but erections did not, the next step is a urology-aware review, not a silent 20 mg booster.
Adcirca 40 mg once daily is a different tadalafil product for pulmonary arterial hypertension. It is not an ED rung and it is not on this cash card. Do not split PAH tablets to invent a weekend dose.
Dates that still govern a 10 mg foil

Meals stay on the plate; five drinks do not
The rate and extent of tadalafil absorption are not influenced by food. That is the cleanest practical split versus sildenafil, where a high-fat meal delays Tmax by about an hour and cuts Cmax by about 29%. A Leith curry does not wreck a 10 mg tadalafil peak the way it wrecks a 50 mg sildenafil peak.
Alcohol is a different line. Ethanol and tadalafil are both vasodilators. Substantial intake - the US label's example is 5 units or more - can raise orthostatic signs: faster pulse, lower standing pressure, dizziness, headache. Tadalafil did not change alcohol levels. Alcohol did not change tadalafil levels. The problem is additive vasodilation.
A 20 mg Saturday tablet plus a long wine list is the pair this desk hears about. The 10 mg lock still carries the same alcohol caution. Daily 2.5 mg does not erase it. It only changes how often the drug is already on board when the drinks start.
Next-morning back pain on the Cialis label
Headache, dyspepsia, back pain, myalgia, nasal congestion, flushing, and limb pain are the common reactions. The delayed back-pain line is the one Viagra compares forget and Cialis forums overplay.
In clinical pharmacology trials, back pain or myalgia generally showed up 12 to 24 hours after the dose and typically resolved within 48 hours. The discomfort was diffuse, bilateral, lower lumbar or gluteal or thigh, and worse when lying down. Most reports were mild or moderate. About 0.5% of on-demand subjects stopped because of it.
That window is why a man can feel fine on Friday night and stiff on Saturday morning and still be on-label. It is also why jumping straight to 20 mg after one weak 10 mg try is a poor plan if myalgia already showed up. Rare events still get named: erection past four hours, sudden vision or hearing loss, NAION talk in men with crowded discs. Those are stop rules.
