The smallest Levitra film is 2.5 mg - here is when it is the dose

Last reviewed · 12 min read · Updated

Levitra is supplied as 2.5, 5, 10, and 20 mg films. For most men the recommended start is 10 mg about 60 minutes before activity. The 2.5 mg tablet is the labelled small start when strong CYP3A4 inhibitors, cobicistat, or certain stacked cautions sit on the chart.

Mira Okonkwo reviewed this frame on 21 August 2026. Capture is the DailyMed CYP table. Layer maps Levitra to vardenafil. Publish will not pretend 2.5 mg is the usual first swallow. Zephyra does not dispense.

Why George Street locks Levitra at 2.5 mg

Open this frame when the bag says vardenafil 2.5 mg, or when a strong 3A4 inhibitor makes 2.5 mg the only legal film.

The 2.5 mg film exists because some charts cannot take the usual 10 mg start. Ritonavir: no more than 2.5 mg in a 72-hour period. Cobicistat: the same 72-hour cap. Indinavir, saquinavir, atazanavir, ketoconazole 400 mg daily, itraconazole 400 mg daily, and clarithromycin: no more than 2.5 mg in 24 hours.

That is a ceiling, not a lifestyle pick. For most men the labelled start is still 10 mg, taken orally as needed, about 60 minutes before sexual activity (UK wording: 25 to 60 minutes). The dose may rise to 20 mg or fall to 5 mg. Maximum frequency is once per day. Sexual stimulation is required.

A Dosage headline that treats 2.5 mg as the default first swallow for a healthy 40-year-old has misread the tablet strengths. A headline that skips 2.5 mg entirely has misread the CYP table. This frame prints the small film because that is the lock, and it prints the usual 10 mg so nobody leaves thinking 2.5 mg is 'the weak one that never works'.

QT screening and onset timing live on the QT glint and the onset layer. Class compare sits on the PDE5 duet.

Orally disintegrating vardenafil is a different product

Staxyn and other ODT vardenafil products are not interchangeable with a 2.5 mg film-coated Levitra tablet on a milligram-for-milligram hunch. Bioavailability and food rules differ. If the chemist substituted an ODT, ask which product landed in the bag before you compare this frame's 2.5 mg cash row to what you swallowed.

Some generic vardenafil tablets contain aspartame. Each 2.5 mg tablet may carry about 0.7 mg phenylalanine. That matters in phenylketonuria. It does not change the CYP cap.

A 2.5 mg vardenafil tablet beside a QT tick

If NHS stock pushed an ODT into the bag without a counselling note, ask which product landed before you compare this frame's 2.5 mg cash row to the swallow. Import strips that say a brand this desk has not priced stay off the quote. We will not invent a second dollar.

What a US window quotes for 2.5 mg times thirty

George Street reprints a sourced GoodRx national row for generic vardenafil 2.5 mg x 30 - the small-start lock - and does not fill the bottle. Twenty-milligram ten-counts are a different board. QT first. GoodRx stays in the caption.

NHS stock gaps sometimes push men from Levitra-brand thinking toward sildenafil or tadalafil. That is formulary logistics, not proof that 2.5 mg failed. Ask the chemist which INN is actually available before ordering strips from abroad.

Most clean charts still start at 10 mg

If there is no strong CYP3A4 inhibitor, no cobicistat, no moderate hepatic impairment, and no stacked alpha-blocker problem, 10 mg is the recommended start. Twenty milligrams is the ceiling. Five milligrams is the usual step down. Two-point-five is the film below that, reserved for the cuts in the next sections.

Elderly men (>=65) have higher exposure: mean Cmax about 34% higher and AUC about 52% higher than younger men. A 5 mg start should be considered. That is not the 2.5 mg lock. It is the geriatric consideration on a clean CYP chart.

Do not swallow a 20 mg film because a mate called 10 mg slow. Peak timing is not the same as peak height. The onset layer exists for that confusion. A second tablet the same day is already a label breach.

A 2.5 mg foil in a man with a clean chart is often a stock or transcription mix, not a lifestyle choice. Ask the chemist which strength was intended before you decide the INN 'never works'. The lock on this frame is the small film when the CYP table demands it, not a campaign to start everyone at 2.5 mg.

A high-fat meal can cut Cmax by 18 to 50 percent

Vardenafil is rapidly absorbed. Absolute bioavailability is about 15%. After a single 20 mg dose in healthy volunteers, Tmax usually sits between 30 minutes and 2 hours, median 60 minutes, fasting. Two food-effect studies showed high-fat meals reduced Cmax by 18% to 50%.

The UK SmPC says Levitra can be taken with or without food, but onset may be delayed with a high-fat meal. That is closer to sildenafil than to tadalafil. A steak-and-chips evening is a timing problem, not proof that 2.5 mg 'never works'.

Total body clearance is about 56 L/h. Terminal half-life of parent and the M1 metabolite is about 4-5 hours on a clean chart. Excretion is mostly as metabolites in faeces (about 91-95%), with 2-6% in urine. On ritonavir the half-life story changes; see the CYP section rather than this paragraph.

2.5 mg in 72 hours is a CYP ceiling

Ritonavir 600 mg twice daily with vardenafil 5 mg produced a 49-fold rise in vardenafil AUC and a 13-fold rise in Cmax. Half-life stretched to about 26 hours. That is why the labelled cap is a single 2.5 mg dose in a 72-hour period, not a nightly 10 mg habit with a 'small tablet' excuse.

Ketoconazole 200 mg daily with vardenafil 5 mg produced a 10-fold AUC rise and a 4-fold Cmax rise; the cap there is 5 mg in 24 hours. Ketoconazole 400 mg daily drops the vardenafil cap to 2.5 mg in 24 hours. Indinavir 800 mg three times daily with vardenafil 10 mg produced a 16-fold AUC rise and a 7-fold Cmax rise; cap 2.5 mg in 24 hours.

Erythromycin and ketoconazole or itraconazole 200 mg daily sit on the 5 mg / 24 hour rung. Cobicistat follows the ritonavir-style 2.5 mg / 72 hour cap. Hide none of these from the chemist. A 2.5 mg foil in the bag is often the label working, not the GP being timid.

The 10 mg and 80 mg QT study still governs 2.5 mg

A thorough QT study in 59 healthy men aged 45-60 measured the interval at one hour, near average peak concentration. Therapeutic 10 mg and supratherapeutic 80 mg (four times the highest recommended dose) both prolonged QTc versus placebo, with moxifloxacin as the active control. Heart rate rose about 5 beats/minute at 10 mg and about 6 at 80 mg.

The 80 mg dose was chosen because it covers concentrations seen when a low vardenafil dose (5 mg) sits with ritonavir 600 mg twice daily. That is the same interaction that forces the 2.5 mg / 72 hour cap. QT talk and the small film are one story, not two rumours.

Men with congenital long QT, or men on class IA or class III anti-arrhythmics, should avoid vardenafil. That is a labelled avoid, not a 'monitor and see' hedge. A normal ECG last year does not clear a new amiodarone start this month. Sudden syncope with palpitations after a dose is 999, not a glint-thread delay.

Cardiac stops that outrank a fast-peak rumour

  • Congenital long QT or class IA/III anti-arrhythmics: avoid vardenafil
  • Ritonavir / cobicistat: 2.5 mg in 72 hours, and the QT study used that concentration band
  • Clarithromycin: 2.5 mg / 24 h and a QT conversation
  • Nitrates and riociguat: still contraindicated at 2.5 mg

A stable alpha-blocker starts at 5 mg, or 2.5 with CYP

If the man is already stable on an alpha-blocker, PDE5 inhibitors start at the lowest recommended dose. For vardenafil that is 5 mg. If a listed CYP3A4 inhibitor is also on the chart, the start becomes 2.5 mg. Do not start both drugs the same week as a home experiment.

Standing dizziness after the first combined evening is a sit-down and call problem, not a reason to add a second 2.5 mg film. Stepwise rises in the alpha-blocker can drop pressure further while vardenafil is on board.

Nitrates remain a hard contraindication at every vardenafil strength, including 2.5 mg. There is no 'tiny film' exception for GTN, isosorbide, or poppers.

A man who started tamsulosin last Tuesday and wants Levitra on Friday is not 'stable on alpha-blocker therapy'. Stable means weeks, not a shared shopping bag. Sit if the first combined evening goes grey. Do not add a second 2.5 mg film to chase the evening.

Age 65 and Child-Pugh B do not default to 2.5 mg

Phase 3 trials included more than 834 elderly men and did not show a different safety story at 5, 10, or 20 mg versus younger men. Exposure is still higher, so a 5 mg start should be considered at >=65. That is one step above the 2.5 mg lock, not the same sentence.

Moderate hepatic impairment (Child-Pugh B): start 5 mg, maximum 10 mg. After a 10 mg dose in that group, Cmax rose about 130% and AUC about 160% versus healthy controls. Severe hepatic impairment is outside casual ED prescribing on this frame.

Severe renal impairment (CrCl under 30 mL/min): a 5 mg start should be considered. Mild-to-moderate renal impairment does not require an automatic cut. None of those sentences replace the CYP table when ritonavir is already on the chart.

QT first, then decide whether 2.5 mg is the film

Levitra's smallest film is 2.5 mg. On a clean chart the start is still 10 mg. On ritonavir or cobicistat the cap is 2.5 mg in 72 hours. High-fat meals cut the peak. Class IA/III anti-arrhythmics and congenital long QT stay off this INN. Nitrates stay banned at every strength.

Torquil, Trinity The chemist gave 2.5 mg. Forum says real Levitra starts at 10. Did they underdose him?

Desk reply

Not if a listed strong CYP3A4 inhibitor, cobicistat, or a stacked alpha-blocker-plus-inhibitor caution is on the chart. Ritonavir and cobicistat cap vardenafil at 2.5 mg in 72 hours. Several other strong inhibitors cap it at 2.5 mg in 24 hours. That foil is often the label working. On a clean chart the recommended start is still 10 mg. Bring the full med list, not a forum screenshot. The onset layer is about timing, not about ignoring a 49-fold AUC rise.

Mhairi, Abbeyhill He wants 20 mg because 2.5 mg 'did nothing' after chips. Is that the next step?

Desk reply

Not from this inbox. On a clean chart the labelled start is 10 mg, not a jump from 2.5 to 20. A high-fat meal can cut vardenafil Cmax by 18% to 50%. Chips plus the smallest film is a food problem until a fasted 10 mg evening - if the CYP table allows 10 mg at all. If ritonavir or cobicistat is on the chart, 20 mg is not the next step. The cap stays 2.5 mg in 72 hours. Read the onset layer for timing and the CYP table on this frame for the film.

Morag, Morningside He is 68 and on tamsulosin. Is 2.5 mg the right first film?

Desk reply

Age >=65: consider 5 mg because Cmax and AUC run about 34% and 52% higher. Stable alpha-blocker: start vardenafil at 5 mg. If a listed CYP3A4 inhibitor is also present, that start becomes 2.5 mg. Two of those facts can land on one chart. All three should not be guessed from a forum. Do not start the alpha-blocker and vardenafil the same week as a home trial. Standing dizziness is a sit-down problem. Nitrates remain banned at 2.5 mg.

Sinead, Newington Child-Pugh B fatty-liver clinic. Can he use 2.5 mg because it is the smallest?

Desk reply

Moderate hepatic impairment starts at 5 mg and tops at 10 mg. After 10 mg in that group, Cmax rose about 130% and AUC about 160%. Two-point-five milligrams is not the automatic hepatic start unless a CYP cap is also in play. Severe hepatic disease is outside this frame's casual ED talk. I will not pick 2.5 versus 5 from an ultrasound nickname. Bring the formal Child-Pugh letter to the prescriber. If a listed CYP inhibitor is also on that letter, the 2.5 mg / 24 hour or / 72 hour cap can sit on top of the hepatic start. That is two cuts, not a reason to skip the visit.

Calum, Leith ECG last year was normal. Do we still worry about QT on 2.5 mg?

Desk reply

A normal ECG does not erase congenital long QT or a new class IA/III anti-arrhythmic. The thorough QT study used 10 mg and 80 mg; the 80 mg arm covers concentrations seen when low-dose vardenafil sits with ritonavir - the same pairing that forces 2.5 mg in 72 hours. I will not clear a 2.5 mg film by email. Bring the ECG date, the drug list, and any faint-without-warning story to the prescriber. Syncope with palpitations after a dose is 999. A normal tracing last spring does not cover a new amiodarone start, and it does not cover a ritonavir pairing that the 80 mg QT arm was built to mimic.

Lewis, Gorgie Chemist swapped in an orodispersible tablet. Same as the 2.5 mg film?

Desk reply

No. Staxyn-type ODT vardenafil is a different product. Food rules and bioavailability are not a milligram-for-milligram copy of a film-coated 2.5 mg Levitra tablet. Ask which product is in the bag before you compare this page's cash row to what he swallowed. If the swap was a stock gap, the next honest comparison may be sildenafil or tadalafil on the PDE5 duet, not a second unverified ODT from abroad.

Angus, Portobello Clarithromycin for a chest infection - hold the 2.5 mg or keep it?

Desk reply

Clarithromycin is on the strong-inhibitor list that caps vardenafil at 2.5 mg in 24 hours, and it is a QT drug. Ask the GP or pharmacist before the next swallow. Many men simply hold the PDE5 until the antibiotic course ends. Azithromycin is not a free substitute you decide in the kitchen. Still ask. Still document. Nitrates stay contraindicated either way.

Bridie, Haymarket He wants the fastest tablet. Is 2.5 mg pointless because it is small?

Desk reply

Small milligram is not slow milligram. Fasting Tmax still sits near an hour. High-fat meals can cut Cmax by 18% to 50%, which feels like 'the tablet did nothing' after a steak. Dose height and food are different problems. If the chart is clean and 2.5 mg was a stock accident, ask the prescriber about the labelled 10 mg start. If the chart holds ritonavir, 2.5 mg in 72 hours is the cap, and 'fastest' is the wrong word. Read the QT glint before anyone chases speed.

Iona, Bruntsfield The 2.5 mg cash band looks expensive. Is that a George Street price?

Desk reply

It is a sourced GoodRx national row for generic vardenafil 2.5 mg x 30, retail and coupon both printed, with official US pharmacy links. GoodRx stays in the caption. We do not invent an Edinburgh street price and we do not fill NHS bags. Mail [email protected] if a cite is wrong. Chest pain, syncope with palpitations, priapism, or sudden vision loss is 999.
