Screen the interval before the 2.5 mg swallow
Levitra's usual labelled start is 10 mg about 60 minutes before activity. Tablets exist at 2.5, 5, 10, and 20 mg. Zephyra's editorial lock is 2.5 mg: lowest available film-coated strength, used when a clinician wants a small first exposure or when CYP3A4 inhibition forces that cap.
A thorough QT study compared therapeutic 10 mg and supratherapeutic 80 mg vardenafil with moxifloxacin. Both vardenafil doses and the control prolonged QTc similarly. A later study of vardenafil plus another QT-active drug showed an additive effect.
Men with congenital long QT, or who take quinidine, procainamide, amiodarone, or sotalol, should avoid vardenafil. A 2.5 mg tablet does not cancel that sentence. Read the parent vardenafil frame for the rest of the ladder.
A clean ECG last year does not cover a new Class III start this month. The 2.5 mg box needs a current list, not a shrine to an old tracing.
Ritonavir-era 2.5 mg every 72 hours is a CYP ceiling. It is not a QT workaround and it is not a nightly habit.
If cardiology bans Levitra and allows another PDE5, write the allowed INN and milligram. Do not keep 2.5 mg 'just in case' beside amiodarone.
Holiday moxifloxacin plus leftover 2.5 mg
Chest infections on holiday are how people add a QT-active antibiotic to a PDE5 that already has a QT study. Pause 2.5 mg for that course unless the prescriber keeps it.
Azithromycin and other macrolides can be CYP and QT stories at once. Show the antibiotic box with the 2.5 mg box.
Do not 'use up' 2.5 mg on the last holiday night because you paid for the strip. A leftover is not a deadline.

If you took both and feel a flutter, you need an ECG, not a second 2.5 mg to see if it settles.
Pack the avoid list in the same pouch as the 2.5 mg so a foreign pharmacist can see it.
When the clean ECG points to another milligram lock
A clean QT and a man who wants two evenings from one swallow belongs on tadalafil 10 mg, not on vardenafil 20 mg.
A clean QT and a restaurant-heavy life may still prefer sildenafil 50 mg with a meal rule. Vardenafil 2.5 mg is not mandatory just because the ECG was fine.
A dirty QT points off vardenafil. It does not automatically bless 20 mg of something else.
Write the ECG date on the 2.5 mg box. Renewals a year later need a new list, not a shrine to last year's tracing.
If the only 2.5 mg reason was ritonavir, say that so nobody 'steps you up' to 10 mg during an antiviral change.
What to put in front of the GP or cardiologist
Current ECG if one exists, the antiarrhythmic list, the HIV or transplant list (CYP3A4), and the exact vardenafil strength in the drawer.
A consumer watch QT number is not a 12-lead. Do not green-light 2.5 mg from a wrist readout.
Scottish locum notes sometimes say 'Levitra' without a strength. Make them write 2.5 mg if that is the agreed small start.
Before the first 2.5 mg
- Name amiodarone, sotalol, quinidine, procainamide out loud.
- Name ritonavir and azoles out loud.
- Chest pain after any PDE5: 999, not a longer QTc debate.
If vardenafil is the wrong molecule
Tadalafil 10 mg PRN and sildenafil 50 mg do not carry the same prominent QT-avoid paragraph, but they still drop blood pressure and still ban nitrates. Cardiology clearance is for sex and for vasodilators, not only for Levitra.
Do not switch to an unlicensed combination strip to 'avoid QT'. Combination PDE5 products are not a cardiac strategy.
If the ECG is fine and the only issue is speed, 2.5 mg can still be the first timed trial. Climb later if the clinician agrees.
Do not test the QT with 20 mg
Twenty milligrams is the labelled ceiling for most men, used after 10 mg was tolerated. It is a terrible first foil in anyone whose ECG is in question.
Supratherapeutic 80 mg was a study dose to cover ritonavir-boosted concentrations. It is not a clinic tablet. Do not invent it.
Onset and food sit in 2.5 mg fast peak, not 20 first. This glint stays on the electrical screen.
The 2.5 mg renewal is a drug-list visit
Amiodarone starts in cardiology and never reaches the PDE5 letter. Renewal is when you catch it.
New sotalol, new moxifloxacin, new ritonavir - any of these change whether 2.5 mg is legal this month.
Bring the ECG date. A tracing from 2022 is not this year's clearance.
If the list is clean, keep 2.5 mg as the first foil. Do not treat a clean renewal as a 20 mg promotion.
If the list is dirty, leave the 2.5 mg on the glass and ask for another INN only if cardiology agrees any PDE5 is acceptable.
Watch screenshots still do not count. Say that to the relative who texts one during the visit.
Write QT-avoid drugs on the 2.5 mg box in pencil. Rub them out when they stop, after the prescriber says you may restart.
A cousin's drowning at 28 is a QT clue, not folklore
Unexplained young sudden death in the family is a reason to get an ECG conversation before any vardenafil, including 2.5 mg. Do not dismiss it as 'he was a swimmer'.
Your own faint after a sprint or a fright belongs on the same list. Congenital long QT is the labelled avoid, not a 2.5 mg special case.
A normal ECG last year does not cover a new sotalol start this month. Repeat the drug list every time the 2.5 mg is renewed.
Hospital QTc numbers use a 12-lead. A watch estimate is not a substitute, as already said, and it still bears repeating when someone texts a screenshot at 21:00.
If cardiology bans vardenafil and allows another PDE5, write that. Do not keep 2.5 mg 'just in case' beside amiodarone.
Who needs an ECG conversation, not a foil
Known long-QT syndrome, unexplained syncope, a family history of sudden death at young ages, or a recent QTc already long on the hospital printout: pick tadalafil 10 mg or sildenafil 50 mg only after someone who owns the cardiology problem agrees any PDE5 is acceptable.
Class IA and Class III antiarrhythmics are named on the Levitra avoid list. Do not 'just use 2.5 mg' beside amiodarone because the milligram looks tiny.
Moxifloxacin and other QT-active antibiotics during a holiday chest infection are a temporary reason to pause vardenafil, including leftover 2.5 mg.

Two different reasons people hold a 2.5 mg box
Reason one: caution. Age over 65, hepatic impairment (Child-Pugh B often starts at 5 mg and caps at 10; Child-Pugh C avoids vardenafil), or a man who fainted on another PDE5. The clinician chooses the lowest tablet.
Reason two: interaction. Ritonavir: no more than 2.5 mg in a 72-hour period. Other strong CYP3A4 inhibitors have similar tight caps. That 2.5 mg is a safety ceiling, not a titrate-up-on-Saturday invitation.
Neither reason is '2.5 mg is safer for QT than 10 mg in congenital long QT'. The avoid list is categorical.
If the QT is unclean, leave the 2.5 mg on the glass
A small tablet is not a small electrical effect in a high-risk heart. Choose another plan with the clinician who reads the ECG.
If the interval is acceptable, keep 2.5 mg as the first foil and do not 'test' with 20 mg. Confirm on the vardenafil frame.
2.5 mg is a small first foil or a ritonavir cap. It is not a long-QT workaround. Congenital long QT and Class IA or III drugs avoid vardenafil entirely.
A clean tracing plus a two-evening life still belongs on tadalafil 10 mg, not on a 20 mg Levitra test. Write the ECG date on the box and renew the drug list every time.
Holiday moxifloxacin plus leftover 2.5 mg is a pause. Flutter after that pair needs an ECG, not a second tablet to see if it settles.
