Lipids and LFTs on a 10 mg isotretinoin start

Last reviewed · 9 min read · Updated

Fasting lipids belong before the first 10 mg and again until the lipid response is known, usually within four weeks. After alcohol, wait at least 36 hours before the draw.

Marked triglyceride rises occurred in about 25 percent of trial patients, HDL fell in about 15 percent, cholesterol rose in about 7 percent. Those figures are not reserved for 40 mg capsules.

Ten milligrams still needs a fasting tube

IPLEDGE is the pregnancy gate. This glint is the blood gate. Pretreatment fasting lipids, then repeats until the pattern is clear - usually by week four.

High-risk groups (diabetes, obesity, heavy alcohol, known lipid disorder, family lipid disease) need more frequent checks if 10 mg proceeds. Uncontrolled hypertriglyceridemia is a reason to stop, not to 'stay on 10 mg because it is small'.

The REMS calendar is iPLEDGE before week one. Parent sheet: isotretinoin frame.

Toast before a 'fasting' tube wastes a 10 mg window. Rebook. Water-only is the usual fast. Alcohol needs 36 hours, counted from the last drink.

UK triglycerides in mmol/L are not the US 800 mg/dL headline. 800 mg/dL is about 9 mmol/L. Do not panic at 2.8 and do not ignore a rise from your own baseline.

Daily paracetamol for a new headache on 10 mg is both an LFT confounder and a reason to ask whether the headache is ordinary dryness or raised pressure.

Book the next draw before the 30-day 10 mg ends. A late tube delays the next capsule more often than a late pill.

Liver enzymes are the other column

LFTs are checked before and during. Hepatitis and enzyme elevations are labelled. A quiet 10 mg does not make a rising ALT a mystery vitamin.

Paracetamol habit, herbal 'skin teas', and viral illness all move LFTs. Bring the list.

Lipid and LFT chips beside isotretinoin 10 mg

Jaundice, dark urine, or severe right-upper-quadrant pain: hold 10 mg and urgent review.

The 36-hour gap is a lab instruction

After drinking, wait at least 36 hours before fasting lipids. A Thursday party and a Friday 08:00 draw is how people look like they failed 10 mg when they failed the fast.

Alcohol also raises triglycerides on its own. 'I only drink on 10 mg weekends' is still a lipid story.

Do not skip the 10 mg the night before labs unless told. The draw is about the drug-plus-you pattern, not a drug holiday.

The night-before script for a 10 mg fasting draw

Last alcohol at least 36 hours before the tube. Last food as they define fasting, usually from midnight. Water yes. Bacon roll no.

Take 10 mg as usual unless they told you to hold. The draw is the pattern, not a drug holiday.

Write last drink time and last food time on a slip in the bag with the form.

If you broke the fast, rebook. A fed lipid on 10 mg is how people get a fake crisis or a fake all-clear.

Night-shift: pick a morning you can actually wake and fast, not the morning after a canteen fry-up.

Bring the 10 mg blister so the form lists the strength.

If diarrhoea from a virus wrecked the week, say so. Illness moves lipids and LFTs.

Book the next draw before you leave phlebotomy if they want a repeat inside the 30-day window.

Keep the PDF. The next locum will ask for a baseline you already paid for.

What the 25 / 15 / 7 percents mean at 10 mg

Trial language: marked triglyceride elevations in about one in four, HDL down in about 15 percent, cholesterol up in about 7 percent. Effects were reversible after stop in those trials. Some people lowered triglycerides by cutting fat, alcohol, and weight, or by reducing dose, while staying on drug.

Triglycerides above 800 mg/dL have been reported. Pancreatitis risk sits on that extreme, not on a mild bump. Still, a rising trend on 10 mg is a clinic fax, not a wait-until-month-four plan.

The 10 mg start may move lipids less than 1 mg/kg - or not. Do not skip the baseline because you expect a quiet capsule.

Book the bloods before the 30-day 10 mg ends

The next iPLEDGE window wants numbers. A late draw delays the next 10 mg more often than a late pill.

Night-shift workers should pick a morning they can actually fast. A post-canteen 'fasting' tube wastes a week.

If you split 10 mg BID, the fast is still a morning fast. Do not skip the evening 10 mg unless told.

Keep PDFs. Locums restart without a baseline when the first hospital email is lost.

A perfect lipid set is how they climb off 10 mg toward mg/kg. A messy set is how they stay or stop. Draw it on purpose.

Paracetamol weeks on 10 mg

Occasional labelled paracetamol for a headache is a different load from daily high-dose paracetamol for a hangover. Tell them your habit before they read the ALT.

Herbal 'liver cleanse' teas are a second variable. Stop them before you blame 10 mg for an enzyme bump.

Viral illness moves LFTs. A week-four draw during flu is a messy 10 mg story.

Alcohol is both a 36-hour lipid problem and an LFT problem. Two reasons to date the last drink.

Do not add ibuprofen stacks for back pain from the gym without naming them. The 10 mg is not the only liver visitor.

When 10 mg should pause for a number

Uncontrolled triglycerides, symptomatic pancreatitis picture, significant hepatitis, or a pregnancy-programme failure: stop and call. Dose reduction is a specialist tool, not a home snap of the 10 mg.

A mildly high triglyceride at week two with a plan (diet, alcohol cut, early repeat) may continue 10 mg. That plan is written.

Do not add a statin from a relative's box to 'protect' a 10 mg course.

How to book the week-four 10 mg labs

Book the bloods before the 30-day supply ends so the next window has numbers. Evening shifts: pick a morning you can fast.

If the 10 mg is split BID, still fast for the lipid tube. The clock of the capsule is not the clock of the draw.

Keep copies. Locums restart 10 mg without a baseline when the first hospital PDF is lost.

  • Baseline fasting lipids and LFTs dated.
  • Week-two to week-four repeat booked.
  • Alcohol-off 36 hours written on the fridge.

Mmol/L versus mg/dL on a 10 mg printout

UK labs often report triglycerides in mmol/L. US labels talk in mg/dL. The 800 mg/dL crisis number is about 9 mmol/L. Do not panic at 2.5 mmol/L as if it were 800.

Do not be casual at 4 mmol/L and rising on 10 mg either. Call with the units they use.

A conversion error is how people stop 10 mg for a breakfast and how people ignore a real spike.

Keep the same lab if you can. Switching hospitals mid-course adds method noise.

Write the unit on the fridge next to the 36-hour alcohol rule.

Draw the lipids. Then decide if 10 mg climbs

A clean week-four lipid set is how a clinician moves from 10 mg toward a weight-based total. A messy set is how they stay at 10 mg or stop.

Confirm holds with the specialist. This glint does not interpret your triglyceride printout.

Draw fasting lipids on purpose. Toast wastes a window. Alcohol counts 36 hours from the last drink, not from 'Saturday'.

Know your units. 2.8 mmol/L is not 800 mg/dL. Rising from your baseline still goes to the specialist. Uncontrolled highs stop 10 mg.

Book the next tube before the 30-day box ends. A perfect week-four set is how they climb off 10 mg toward mg/kg. A messy set is how they stay or stop.

A new statin or a new oral contraceptive can move lipids and LFTs. Name both at the draw. Do not add a supplement 'for the liver' the week of the tube.

Night-shift fasting is still eight hours without calories. Black coffee only if the lab allows it. A 04:00 kebab then a 09:00 lipid is a wasted 10 mg month.

Pregnancy tests and lipids share a calendar, not a fridge. A lipid-only visit does not reopen a closed iPLEDGE window. Both must be current before the next 10 mg box.

A GP who repeats 'your LFTs are fine' without the number still leaves you without a baseline. Ask for the printout. Week-eight 10 mg decisions need last month's digits, not a vibe.

Glint thread

Hamish ate a roll before the 'fasting' lipids on 10 mg. The number was his breakfast. Educational only. Abdominal crisis or jaundice: urgent care.

Hamish Do I really fast for lipids if I only take 10 mg?

Desk reply

Yes. Pretreatment and follow-up lipids are fasting. Ten milligrams does not change the tube. After alcohol, wait 36 hours. A non-fasting draw wastes a window and can look like a 10 mg failure. Rebook rather than start a diet panic on a fed sample.

Morag Triglycerides went from 1.8 to 3.1 mmol on 10 mg. Stop?

Desk reply

That is a rise, not the 800 mg/dL crisis number (about 9 mmol/L). Call the specialist with the units they use. They may cut alcohol and fat, repeat early, or hold 10 mg. Do not stop-start on a forum conversion. Do not add a borrowed statin.

Morag Daily paracetamol for headaches since 10 mg started. Mention it?

Desk reply

Yes. Headache on isotretinoin can be ordinary dryness or a raised-pressure warning. Daily paracetamol also muddies LFTs. Call about the headache itself - severe headache with nausea or vision change is urgent, not a paracetamol titration. Bring the 10 mg blister and the paracetamol count.

Beathag Can I climb from 10 mg to 40 mg if week-four lipids are perfect?

Desk reply

That is a specialist titration toward the mg/kg target, not a home climb. Perfect lipids help the case. iPLEDGE windows, pregnancy tests, and the next 30-day count still gate the new strength. Do not swallow four 10 mg to invent 40 mg the night you see a good PDF.

Niall UK result 2.8 mmol/L triglycerides on 10 mg. Is that the 800 number?

Desk reply

No. 800 mg/dL is about 9 mmol/L. 2.8 mmol/L is a modest number, not the pancreatitis headline. Still send it to the specialist if it rose from your baseline. Do not stop 10 mg on a forum conversion and do not ignore a rising trend. Bring units. Fasting and the 36-hour alcohol gap still apply to the next draw.

Fiona ALT is 72 on 10 mg. Continue to the dance?

Desk reply

An ALT of 72 needs the clinician who knows your baseline. It is not a dance decision. Hold if they say hold. Jaundice or pain: urgent, not after the dance. Bring the full LFT panel, not one screenshot number.

Angus I ate toast, then remembered the fasting 10 mg lipids. Still go?

Desk reply

Rebook. Toast is food. A fed tube wastes the window and can look like a 10 mg lipid failure. Water-only is the usual fast. Tell them you ate rather than hoping it 'will not matter'. Keep 10 mg as prescribed unless they hold it. Alcohol 36-hour rule still applies to the new morning.

Ewan Diabetic. Extra glucose checks on 10 mg?

Desk reply

Labels ask for more frequent lipids and/or glucose in diabetes, obesity, and familial lipid disease. Ten milligrams does not exempt you. Ask for a written glucose plan the same week as the lipid plan. Uncontrolled sugars plus rising triglycerides is a stop conversation.

Callum I drank Saturday. Bloods Monday 9 a.m. - 36 hours yet?

Desk reply

Saturday night to Monday 09:00 can be under 36 hours if you were still drinking at midnight. Count from the last drink, not from 'Saturday'. If you are short, delay the draw. A late lipid is better than a false 10 mg scare. See the isotretinoin frame.

Struan What do I write on the lab form?

Desk reply

Isotretinoin 10 mg, fasting lipids and LFTs, last alcohol [time], last dose [time]. That line stops a random non-fasting add-on and keeps the lock visible.
